in the mall parking lot in front of Chick-fil-a. We will be traveling to the Georgia Baptist Conference Center in Toccoa, Ga to attend a GoTell Ministries Student Camp lead by Rick Gage. After submitting the form an email will be sent with a link to download the completed form. Please, print 3 copies. One will be for Connect Church (due by June 10) One will be for the Go Tell Nurses Office (due by June 10) One will be for your records |

Step Three - Background Check
|
Go to the Laurel Police Department and have a background check run. Mark through your SS#, make a copy to give to the church ASAP.
